Who Can Access a Support at Home (SAH)?
Home Care Packages are designed to support older Australians to live independently at home for as long as possible.
You may be eligible if you:
✅ Are 65 years or older (or 50+ for Aboriginal and Torres Strait Islander people)
✅ Need help with daily tasks, personal care, or staying safe at home
✅ Are an Australian citizen or permanent resident
✅ Have completed an ACAT assessment confirming your care needs

Our Services
We offer tailored Support At Home (SAH) services to help older Australians live safely, comfortably, and independently at home. Our services include:
- 🧹 Domestic Assistance – cleaning, laundry, and household tasks
- 🧑⚕️ Personal Care – showering, dressing, grooming
- 🚗 Transport – to medical appointments, shopping, and community outings
- 🏠 Home Safety Support – falls prevention, equipment, and home modifications
- 💬 Allied Health Services –Multidisciplinary Team (MDT) Allied Health Services – including Occupational Therapy, Speech Pathology, Physiotherapy, and other relevant disciplines
- 🧭 Care Management – ongoing support to coordinate and adjust your care plan as your needs change
